Testlio’s 2026 Software Quality Report shows that 34% of clients who work with the company uncover at least one critical or high-severity defect in the very first testing cycle. Most often, these issues are found in functional bugs (44%, like broken logins and onboarding flows) and payment failures (22%), with the remainder split across UX/usability (20%) and other areas like localization, accessibility, and regression (14%).

AI assistants posed an even greater risk, with 77% of failures being significant enough to damage user trust and product integrity.

These high failure rates highlight a growing vulnerability in the modern software lifecycle.
